WELCOME ABOARD PROJECT FLAGSHIP!
This page gives you a list of the documents required to complete your application to the
FLAGSHIP program, together with links to download printable versions of the documents,
which you should then print, fill in, and mail back to the Teacher Education Department.
The steps required to get aboard Project FLAGSHIP are:
- FAFSA (Free Application for Federal Student Aid): FAFSA (Free Application for
Federal
Student Aid) IS REQUIRED FOR ALL STUDENTS APPLYING FOR THE PROJECT
FLAGSHIP GRANT.
The best way to apply is to go to the FAFSA web site (
Click here) and fill out the application online.
You will need the school code for Lehigh Carbon Community College, which is #006810. A
pin number is required for electronic signature on line, or the signature page at the end of the
application must be printed, signed, dated and mailed to the processor to validate the
application. If you need assistance, you may call the toll-free number, 1-800-433-3243.
Dependent students must include parental income information and signatures on the FAFSA
form. Paper FAFSA forms are available at the Office of Financial Aid, however, there is a
four-six week processing time for a paper application. If additional information is needed by the
Office of Financial Aid after the form is filed, the student must reply promptly.
Note that Applications for the grant will not be considered without a complete FAFSA form.
There are no exceptions to this requirement.
Project FLAGSHIP Special Education A.A.S. Degree Request Form. Download this
form from here.
Please complete this form and include an essay of 500 words or less.
Project FLAGSHIP Scholarship and Service Obligation Agreement. Download this form from
here . Carefully review, sign
and date this form.
LCCC application for enrollment. Go here and click on "Printable
application and residency form" to print a copy of the LCCC application.
The $30.00 LCCC application fee is waived under the Project FLAGSHIP grant.
NOTE: DO NOT select the "Apply Online" option. The college application must
be routed through the Teacher Education Department in order for your application fee to
be waived.
NOTE: If you are currently a Lehigh Carbon Community College student, you do not have
to complete the application.

Statement of Residency. The college application that you downloaded in step 4 includes
this form.
If you are a first-time Lehigh Carbon Community College student, you must complete this
form.
NOTE: You do not have to complete this form if you are currently a Lehigh Carbon
Community College student.
Student Change of Information Form: This form is only completed in certain circumstances,
including
- If you are already enrolled in a program and you are changing your major
- If you are a returning LCCC student changing your last name and/or contact
information.

Study the Program of Study for the Special Education A.A.S. degree

and review the
current semester schedule for the course offerings.

Schedule an appointment with an advisor
(Student Services Department, 610-799-1137), and then complete the Registration Card for the
upcoming semester.
Once all of these steps are complete and the Teacher Education Department receives the
results based upon your FAFSA (Financial Aid) Application, you will be contacted to schedule an
appointment with the Director of Project Flagship. After that interview, you will be notified on the
status of your scholarship award. Please keep in mind that your award will be monitored and
adjusted according to your academic GPA and attendance.
